The Community Home Support Program (CHSP) helps older Australians access entry level support to live independently and safely at home. This role, as part of the care industry, is in a fast-growing sector with opportunities for individuals who are looking for ongoing, rewarding work where they can give back to their local communities. We have an exciting opportunity within both our Clare and Kadina offices (or a mixture of both) supporting our Mid North and Yorke Peninsula clients where we are experiencing growth for the services.
This role is Permanent Part Time position, which may be three days per week in each region, or five days per week across both regions for the right candidate.
Carers and Disability Link are looking for someone who has excellent:
- Communication Skills both written and verbal
- Ability to identify opportunities for growth
- Compassion for clients and the community you live in
- Time management skills
- Computer Skills (including CRMs and Microsoft Office)
- Understanding of the CHSP Manual, and My Aged Care Reporting knowledge or willingness to learn.
These skills will set you up to excel in the role as a Direct Care Services Coordinator (CHSP) at CADL. As a part of the role you will:
- Undertake CHSP participant engagement through service agreements and care plans
- Have the ability to work collaboratively with other aged care service providers
- Coordinate Supports and local groups for CHSP clients
- Work independently and as a part of a team, planning and building internal and external relationships
- Have an understanding of the potential needs, barriers and opportunities of persons who are frail and aged and their primary carer/s
- Have the ability to work with our clients to meet personal goals, client driven outcomes and the organisations targets under CHSP funding agreements
- Have the ability to provide professional services with dignity and respect.
Evidence of qualifications in disability, aged care or community services or equivalent is desirable but not essential. CADL are open to applications from all industries.
