About the role:
To provide high-quality clinical care and support to clients while leading and mentoring care staff, ensuring the delivery of safe, effective, and compassionate care. The role involves overseeing care plans, coordinating with healthcare professionals, and maintaining compliance with clinical standards and regulations.
Mentor a dedicated team of professionals in ensuring Barunga Villages Home Care Package services are not only sustainable but aligned with the new program changes. You’ll work closely with clinical and operational staff to ensure the highest standards of care are met and will be responsible for coordinating a consumer caseload.
Responsibilities of the role:
Assessment, Planning and Coordination
- Coordinate a caseload of consumers with extremely complex health and social care needs and apply evidence-based practice and person-centered principles to assess, plan, implement and evaluate interventions.
- Provide clinical assessment, planning and intervention of consumer clinical needs.
- Liaise with external health services and assessment organisations to promote access to appropriate services, and management of each consumer’s needs.
- Monitor and manage consumer revenue against the budget, identifying opportunities for package utilisation that support each consumers goals, needs and preferences.
- Recognise change in the condition of the consumer and alter care plans and risk management strategies accordingly or escalate to external health services, as required.

- Provide information and advocacy to consumers and their representatives and support informal care arrangements that facilitate goal achievement.
Clinical Practice & Leadership
- Work as an autonomous practitioner and display appropriate problem solving, analytical and judgement skills.
- Adhere to the AHPRA standards and ethics and make appropriate clinical observations and keep accurate records in accordance with professional standards.
- Provide Clinical Leadership and delegation of tasks, providing support and feedback to clinical and non-clinical staff.
- Contribute to implementation of local clinical governance initiatives for the service.
Quality & Safety
- Investigate consumer incidents and feedback to ensure timely resolution and service delivery meets the highest standards.
- Ensure compliance with relevant policies, procedures, and regulatory requirements, including Aged Care Quality Standards and HCP guidelines.
- Identify and actively participate in continual improvement initiatives within program areas.
- Inspect work areas and equipment in line with Occupational Safety and Health policies and procedures.
Senior responsibilities
- Mentor and support a high-performing and engaged team of clinicians, and care partners.
- Be a key player in Barunga Villages response to the Support at Home Program reform, ensuring the team is equipped to meet the new legislative, regulatory, and operational requirements.
- Assist in the planning and delivery of care partner inductions, staff meetings, supervision, and training events/activities.
- Guide the operational delivery of home care packages and CHSP services, ensuring that consumer needs and satisfaction remains at the forefront of our service delivery.
Organisational Lead for
- Assistive Technology and Home Modifications.
- Restorative Care Pathway.
- End of Life Pathway.
About you:
Essential qualifications and experience
- Bachelor’s degree in Nursing.
- Minimum of four years’ experience in Home Care programs in a level 2 role.
- Current registration with the Nursing and Midwifery Board of Australia.
- Proven experience in mentoring teams within the healthcare, aged care, or disability sectors.
- In-depth understanding of HCP guidelines, CHSP Guidelines, Aged Care Quality Standards, and compliance requirements.
Desirable qualifications and experience
- Proficiency in budgeting and financial management within the context of home care services.
- Understanding of the Support at Home Reforms.
- Strong organisational skills with the ability to manage competing priorities and deliver results
- Excellent communication skills, both verbal and written.
- Strong interpersonal skills with the ability to build trust and resolve consumer concerns.
